Student Planned Absence Form

Although we are not able to routinely alter monthly tuition billing, on rare occasions students are able to request temporary adjustments from monthly billing to singles rate billing. If you plan to miss more than one lesson in a single month, it might make sense to request a switch. This form is a request to exclude lesson(s) you plan to miss. If approved, the lessons you will be attending will be adjusted to singles rate ($45.65) which will allow you to hold your time slot and return to monthly billing the following month. We are not able to exclude an entire month. Students needing to miss an entire month will be removed from the roster and invited to call the shop desk when ready to return to lessons.

Requests must be made 1-2 weeks prior to the month in question. We contract with our teachers on a month-to-month basis and are not able to adjust student lessons prior to placing teacher’s schedules in our system. All requests must be in by the 25th of the prior month in order to be considered for adjustment. Once tuition is run, we do not adjust billing to accommodate late requests.

Our staff will ALWAYS contact you to confirm we have received your request. If billing changes are approved, billing will be run by hand at that time (or shortly thereafter) and you will receive confirmation of changes. Note: If your instructor plans to teach all 5 lessons in a 5 week month and you request one week off, we will exclude one lesson and retain your monthly tuition status without adjusting to singles rate. If your instructor only plans to teach 4 weeks, this “freebie” skip does not apply.
